GIFT CERTIFICATE - ONLINE ORDER form

TERMS & CONDITIONS
(1) All Gift Certificates issued by KamenRider School can only be used for motorcycle training.
(2) All Gift Certificates cannot be redeemed for cash and have no cash value.
(3) All Gift Certificates will expire after one (1) year from the issued date.
(4) All Gift Certificates cannot be refunded after 30 days of the purchase date. Only the original purchaser can cancel their Gift Certificate within 30 days of the purchase date. Fifty dollars ($50.00) administration fee will be deducted and the balance will be refunded.
(5) All Gift Certificates can only be applied to the specific motorcycle training package, unless not specified.
(6) Gift Certificate with a specific amount can be used as deposit or part of total payment. When the total amount exceeds the amount remaining on the Gift Certificate, the recipient will be required to pay for the balance.
(7) All Gift Certificates can be transferred to others with the agreement of the original purchaser.
(8) All Gift Certificates will be issued in electronic form. The recipient can redeem the Gift Certificate by printing out or using the Certificate's number.
(9) All Gift Certificates will be sent out by email within 5 working days.
(10) All Gift Certificates resale is prohibited.
